Coaching Consent Form
We ask all participants to complete the consent form before taking part in one of our Try Curling, Beginner or other coaching sessions. This will be kept on record and you only have to fill this out once during a season. So if you fill it in for a Try Curling class, then it will also cover you for any further session that you attend for the rest of the current season.
This form should be completed by each participant with their own details, not by a group leader for everyone. Parents or carers of children should use their own email and mobile number and sign on their behalf if this is more appropriate.
Your contact details will only be used for the purpose of communicating with you about curling sessions and will not be passed on to any other group without your permission. If you tell us you no longer wish to hear from us we will delete your contact details.
Please read the following previous experience and safety information as you will asked to confirm these in order to participate.

Safety Information
Whilst we ensure that our activities are as safe as possible, unfortunately accidents in sports do happen.
We will
- Ensure our coaches are qualified, DBS checked, and child protection trained.
- Provide all the equipment needed and check it regularly.
- Offer a protective headband to wear on the ice. This is recommended, but not compulsory.
- Ensure activities are age appropriate and appropriate to participants capabilities.
- Aim to make the sessions enjoyable for everyone.
We ask that participants
- Tell us or the coach of any physical disabilities or restrictions they have.
- Follow the instructions of the coach.
- Are kind and polite to the coaches and other participants.
- Understand that they voluntarily elect to participate, while accepting that there are certain risks.
We apologise that the form does not look very good just now. This will be fixed, but it is fully functions.
